Update to Form I-693, Report of Medical Examination and Vaccination Record. New edition dated 03/30/15. Starting 07/27/15, USCIS will only accept the 3/30/15 edition. USCIS will not accept the 01/15/13 or 10/11/11 editions on or after 07/26/15.
