AUTHORITIES: The information requested on this form is collected under the Immigration and Nationality Act, 8 U.S.C. § 1182(d)(5).

PURPOSE: The primary purpose for providing the requested information on this application is to determine your compliance with the conditions of your parole.

DISCLOSURE: The information you provide is required as a condition of your parole. Failure to provide this information may result in termination of your parole.

ROUTINE USES: DHS may, where allowable under relevant confidentiality provisions, share the information and any additional requested evidence you provide on this form with other Federal, state, local, and foreign government agencies and authorized organizations. DHS follows approved routine uses, as described in the associated published system of records notices [DHS/USCIS/ICE/CBP-001 Alien File, Index, and National File Tracking System; DHS/CBP-006 Automated Targeting System] and the published privacy impact assessments [DHS/USCIS/PIA- 061 Benefit Request Intake Process; DHS/CBP/PIA-006 Automated Targeting System (ATS)], which can be found at www.dhs.gov/privacy. The Employment Authorization number is the same as your Alien Registration Number (A-Number).

The Employment Authorization Number may be located on the front or back of the card, depending on when the card was issued.

Unexpired I-94s issued in the current numeric-only format will continue to be valid until the Admit Until Date printed on the paper I-94 and/or the date displayed on the public I-94 website.
